Appointments

Most sessions last 50 minutes. We do offer longer, extended sessions. Please ask for more information about our Intensive Counseling Program. Changes in appointments will be accommodated as scheduling permits, however, you will be charged the full session fee if you cancel within 24 hours of your appointment time or fail to show for your scheduled appointment. Group sessions are also subject to these same conditions. Please note that missed appointments or late cancelled appointments with less than 24 hours notice are not billable to insurance and will be charged to your account in full.

Billing Information and Payment Options

Our policy is to receive payment at the time of service. In the event we are submitting claims to your insurance company, you still need to make your "patient portion" or co-payment at the time of service. Payment can be made with check, cash, Visa, MasterCard, American Express, Discover.

THE CENTER, INC. may accept assignment of any insurance benefits. Having applicable health insurance in no way relieves you of financial responsibility for any portion of payment and any balance owing is your responsibility whether insurance pays or not. Funds received from insurance will be applied directly to the account. THE CENTER, INC. will wait up to a maximum of 45 days to receive payment from a third party payor, after which time, if payment has not been received, the balance will immediately become your responsibility.

If pre-authorization is a requirement for insurance reimbursement, you may be required to secure initial and subsequent authorizations from the required source, according to the requirement of  your insurance. THE CENTER, INC. is not responsible for services performed which may be unauthorized or not covered by any third party payor. THE CENTER, INC. agrees only to perform professional services that you request and schedule.

If the patient is a dependent child, the parent who brings the child in for treatment is responsible, for payment at the time of service.

Billing statements are prepared and sent out monthly. We do not charge interest on accounts, however a re-billing fee of $15.00 per month will be added to any unpaid balance remaining after 60 days. There will be a $30.00 service charge for returned checks.

In the event that the insurance carrier rejects your claim or the amount reimbursed is less than expected, we ask that you make payment arrangements with the Financial Services Dept. for the balance owing on your account. If the insurance carrier pays more than expected, a reimbursement will be promptly made to you. It is important to notify our office if your insurance carrier changes.

Be sure and inquire about our very special "Enhance Patient Finance" program and how affordable we can make it for you to come and attend your very own personal Intensive Eating Disorder, Depression, Relationship Issues, or Sexual Addiction program for a week, 2 weeks, 3 weeks, 4 weeks, or what ever time is needed.

Convidentiality

Please remember that medical records are confidential. No information will be released without your permission. Your doctor or counselor is prepared to discuss legal exceptions, should you have any questions.

Bookstore

For your convenience we have a bookstore at each office, featuring sound family resources with a wide variety of titles. We also have available nutritional supplements and products. These items are available for purchase at The Center of Counseling & Health Resources, Inc. and on this website.
